If so, please bring this written authorization and your medical insurance card with you to your appointment.

For members of participating insurance plans and Medicare, Missouri Baptist Medical Center will file your claim for you.

• The van is equipped with digital technology and is accredited by the Food and Drug Administration
• It appears at Schnucks stores, area corporations, community sites, and clinics
• Most insurance plans and Medicare cover this screening once a year; the fee can be directly billed to the insurance company or Medicare
• Women with limited insurance or no insurance should call 314.454.8466 to see if they qualify for a mammogram at no cost
